actually, WordPress does not require the index.php… hardly any use it though it is valid… after removing the index.php from your permalink and updating, you need to make sure you clear your browser cache before trying to browser your site… have you done that?

Well with the 'day and name' structure that 'index.php' should just not be there. It is invalid and unecessary. Certainly I can run my site with 'day and name' successfully. Surely when you selected 'day and name' in the WP Permalinks admin screen it didn't out the 'index.php' in the middle did it? It should not have.
The correct permalink for the forum using the 'day and name' is simply the name of the slug - so just 'forum'…
Are you using any plugins that may mess with the permalink structure?

How do I clear caches?
Only plugin I had which potentially messed with Forum was Google Analytics but that was disabled at the start!

Depends which browser you use but with Firefox it is underc Options/Preferences > Advanced Tab > Offline Storage.
But sorting out your permalink problem is the most important item.

danstorey,
Thanks for the account info…
After taking a look, the issue is that your web host provider does not have apache mod_rewrite activated on your server… I would contact your host and ask them to turn on apache mod rewrite or see if you can do it from within your control panel… the rewrite rules viewer plugin you have installed clearly indicates that mod_rewrite cannot be found (ie not active)…
Or perhaps your server is running IIS?
To be honest, I am a bit surprised and did not know that WP dealt with attempts to use pretty permalinks without mod_rewrite in this manner… I just thought it was required… pretty nifty actually… we may need to consider seeing if we can get our permalinks working with this previously thought unsupported setup…

danstorey,
I do have a mod to our rewrite code that should work with this PATHINFO style permalinks… would you be willing to try the file on your server (before doing my recommendations above) and see if it fixes your permalinks issue? If so, let me know and I can email you the single file to upload…
